Sixty-two patients with a diagnosis of Philadelphia chromosome-positive myeloproliferative disorder have been studied by the triple-label immunofluorescence technique. The cultures were performed on marrow aspirate smears immediately after collection and the immunolabeling was performed with monoclonal antibodies directed against the lymphocyte antigen specific for both the clonotypic and nonclonotypic class of T cells. The abnormal cells were identified on smear sections, both in the presence and absence of culture. In all cases, the abnormal cells demonstrated a greater than normal concentration of clonotypic T cells (ALC = 207), greater than normal percentages of LGL-type cells (LCF = 18%) and a great majority of these cells with the clonotypic features of T cells (ALCT = 96%). The clonotypic T cells did not express an HLA-DR antigen, which is more characteristic of activated memory T cells. The finding of LCF and ALCT on bone marrow smears strongly suggests an immunologic pathogenesis of the hematologic manifestations of the myeloproliferative disorder.
